Before Your Appointment
To make your visit as smooth as possible, here's what we recommend bringing and preparing:
A valid photo ID
Your vision and medical insurance cards (if applicable)
A list of current medications or eye drops
Your most recent glasses or contact lens prescription
Any previous eye surgery or treatment records
If you're coming in for a vision correction procedure or consultation (LASIK, SMILE, EVO ICL), please remove contact lenses at least 24 hours before your appointment. This helps us get the most accurate eye measurements.
For your convenience, you can download our new patient forms and notices, complete them at home, and bring them to your first visit. This makes it easy to fill them out at your own pace – and saves you time on the day of your appointment.

Your first appointment is more than just an eye exam, it’s your vision discovery session. Here's what happens:
Comprehensive Testing
We’ll perform advanced scans and diagnostic imaging to understand your eye health and vision needs.
Meet Dr. Ashraf
You’ll sit down one-on-one with Dr. Ashraf to review your results, discuss your goals, and determine which treatment (if any) is right for you.
Customized Recommendations
Whether LASIK, SMILE, PRK, or another solution, Dr. Ashraf will walk you through your options and answer all your questions.
Transparent Pricing & Scheduling
We’ll review financing options and next steps. We provide clear information so you can make the best decision for your eyes.
